As discussed, the tentative start date will be (start date). Your annual (hourly/monthly) base salary will be $X payable every two weeks.
The following benefits will be added to your base salary:
You will be enrolled in the company bonus program. Bonus payments are based on individual performance and the global performance of the enterprise.
You will receive X days of annual vacation in 2008 (pro-rated for the current vacation year, according to the terms of our vacation policy). As of January 1st 2009, you will be eligible for X days of annual vacation.
As a permanent (contractual) employee you will be eligible to participate in the Company Names group benefits plan (health, life, disability, dental and vision insurance). Employee contribution to payment for benefit plans is determined annually. Please consult the company website to learn more on this Group benefit plan.
This offer is conditional on:
Your success completion of the six (6) month probation period, after which point you will become a permanent employee. Satisfactory results from your pre-employment medical evaluation. Please fill out the attached questionnaire and return it directly to XXX within 5 days.
